You will receive online intake forms once your appointment has been scheduled. Please give yourself time to complete the forms once you begin, as they will not store answered questions if you stop and try to complete later. If you prefer to complete hand written forms, you can download them here, or come 15 minutes prior to your appointment time to complete them. Cancellation and Lateness Policy
If you are unable to keep your appointment for any reason, please call the office as soon as possible, preferrably at least 4 hours prior to your appointment time so that someone on the wait list may fill your slot. You may also cancel online. Canceling less than 24 hours ahead of your appointment time will result in a fee of 50.00. This fee will be waived if we are able to fill the slot. You will be charged the full amount for the scheduled service for failure to show up without cancelling your appointment.
Please arrive on-time for your appointment, arriving late will take away from your allotted appointment time.
